Shoulder pain comes in many different forms and one of the most common is rotator cuff syndrome. The rotator cuff is a group of four important muscles that both support the shoulder and rotate the arm both clockwise and counter-clockwise. Injury to the rotator cuff can be the result of either trauma (like sprains, muscle strains, separations or dislocations) or chronic, repetitive strain.
There are several different factors that can contribute to the pain of rotator cuff syndrome. Inflammation, muscle spasm, soft tissue adhesion and fibrosis, mechanical joint irritation, poor posture, nerve irritation and muscle weakness are some of the more crucial factors involved with rotator cuff pain. Often when prior treatment has proven unsuccessful, it may be due to not having one or more of these factors properly addressed.
Chronic rotator cuff pain is often the end result of a vicious circle that starts with irritated or injured tissue. This leads to inflammation and muscle spasm, which in turn causes not only nerve irritation and pain but more importantly results in the formation of soft tissue adhesions. These adhesions scar-up the tissue and lead to dysfunctional soft tissues, which are more easily irritated or injured… and the whole cycle continues indefinitely.
In the case of rotator cuff tendonitis, the inflammation and soft tissue adhesions prevent the smooth movement of the shoulder tendons causing friction and further irritation, inflammation and pain in the tissues. Additionally, poor posture can increase the mechanical strain placed on the tendons and muscles themselves.
In the case of frozen shoulder, the ligaments or muscles surrounding the shoulder joint may become inflamed with a large amount of adhesion and fibrosis forming in the ligaments and muscles. These two factors prevent the shoulder joint from moving normally without pain.
